About Dr. Mendiolaza

Dr. Mendiolaza offers his high level of cardiovascular care all over the United States as he holds 23 licenses throughout our nation, holding medical licenses in 27 states and he is currently at the prestigious Heart and Vascular Institute at The University of Pittsburgh Medical Center, MD, where he is providing his expertise in structural heart disease and cardiac imaging using all modalities ( from Cardiac CTA,–Cardiac Computed Tomography, Cardiac MRI, which Cardiac Magnetic Resonance, 3 D echo —which entails a three-dimensional echocardiography and 3 D Echo TEE, (trans-esophageal echocardiography) both state of the art that goes beyond the traditional standard 2 Dimensional echocardiogram that is performed throughout the country and around the world, he adds to such studies novel compelling features such STRAIN echocardiography and mechanical dispersion for better and most accurate information of the heart function and contractility.

In addition, He uses ICE which encompasses Intra Coronary Echocardiography—a highly sophisticated procedure—Intracardiac echocardiography (ICE) can be useful to provide focused real-time images of the endocardial surfaces critical for positioning of catheters, establishing catheter tip–tissue contact, and monitor energy delivery in the beating heart. This procedure Intracardiac Echocardiography (ICE) is a new catheter imaging technology often used to assist closure procedures for atrial septal defects (ASD), patent foramen ovale (PFO), and ventricular septal defects (VSD), which are complex congenital heart disease —using his expertise non invasively percutaneously via vascular access instead of open-heart surgical procedures performed a few years ago.

ALL OF THESE TECHNIQUES ARE PERFORMED without open heart surgery and via vascular access and percutaneously such as for Transcutaneous Aortic Valve Replacement known as TAVR, for mitral valve repair or replacement known as TEER ( MITRA CLIPS) which is the same concept for aortic valve disorders. The same occurs for the tricuspid and pulmonary valve.

In addition, he applies such sophisticated techniques for imaging the heart prior to performing the Interatrial SEPTAL OCCLUDERS FOR PFO (congenital heart defect or Patent Foramen Ovale which entails the presence of a “hole in the heart” therefore mixing the heart from oxygenated to non-oxygenated and may lead to strokes), new ACC ( American College of Cardiology) clinical guidelines which encompasses sealing such structure with a closure device just like placing a “cork in a bottle” sort of speak.

Thus, he also applies such imaging modalities for TRICUSPID VALVES, PULMONARY VALVES plus CLOSURE OF LEFT ATRIAL APPENDAGE, called LAAO for patients with an arrhythmia known as atrial fibrillation (irregular heartbeats and leading to strokes), this entails to seal the left atrial appendage of the heart, which is the area where clots forms and therefore committing the patient to take anticoagulants blood thinners however as part of the new guidelines released November 2023, now this procedure is highly recommended by the new ACC (American College of Cardiology)such technique allows cardiologists sealing such structure with a closure device just like placing a “cork in a bottle” sort of speak and therefore avoiding the perils of using blood thinners plus open heart surgery (as it is performed non invasively).

Moreover, he performs and is trained and certified in all vascular and peripherals procedures a field of vascular surgeons and not cardiologists nevertheless, he uses several techniques such as DSA–Digital Subtraction Angiography, which Provides an image of the blood vessels in the brain to detect a problem with blood flow. The procedure involves inserting a catheter (a small, thin tube) into an artery in the leg and passing it up to the blood vessels in the brain. Digital subtraction angiography (DSA) is a diagnostic procedure to view the inner surface of blood vessels (also known as lumen). It can be used to view arteries, veins, and heart chambers. DSA is a fluoroscopic technique (a technique that captures continuous images) that uses complex, computerized X-ray machines. (for instance brain aneurysms and such)

Therefore, he uses transcranial Dopplers for detecting PFOs, this technique entails a diagnostic test to measure the blood flow to and within the brain. Also, he uses this modality to detect PFO ( small holes in the heart (such as Patent Foramen Ovalle).

He is also now applying the novelty F-fluorodeoxyglucose positron emission tomography with computed tomography (18F-FDG-PET/CT) for infective endocarditis which is a new state-of-the-art technique to ascertain the presence of infective valves named endocarditis all of these to name just name a few.

Finally, he is the former, Director of Nuclear Cardiology, at Yakima Valley Memorial Hospital in Yakima, WA
He provides all Non-Invasive Cardiology,  all noninvasive Cardiovascular procedures, and all cardiac consultations and cardiovascular clinical management and prevention. All non-invasive cardiovascular procedures.

He has recently been re-certified as a level III in Cardiac Computed tomography, which is the maximum level of expertise given in the US and all over the world.

He and his concierge practice covers almost 27 states of the country and his home-based practice is located in sunny Naples, Florida.
